Parent Survey
If you have are a human being (no bots please!), have been pregnant or given birth within the past 12 months and are over the age of 18, we are interested in hearing about your experiences with health care.
This survey includes 17 multiple-choice questions and at least three long answer questions. You may respond by recording your voice or by typing your answer.
Your identity will be kept confidential when we report on our findings.
We estimate that this should take anywhere between 25 minutes and 1 hour.
If you finish the survey, you will be given a $75 incentive as a thank you for your time and effort.
